The Short Answer

Most orthodontic treatment takes between 12 and 24 months. Some simpler cases finish in 6 to 9 months. Some complex cases — particularly those involving jaw discrepancies, severe crowding, or adults with denser bone — can take up to 36 months.

But these are averages. What matters is your specific case.

The most important thing I can tell you: treatment time is a clinical estimate, not a guarantee. Any orthodontist who gives you a fixed end date at the first consultation without detailed records is guessing. A proper timeline requires X-rays, photos, study models, and a thorough clinical assessment.

What Affects How Long Treatment Takes?

Several factors influence your total treatment duration:

  1. Complexity of your case
    • Minor spacing or crowding: 6–12 months
    • Moderate crowding or bite issues: 12–18 months
    • Severe crowding, significant bite problems, jaw discrepancies: 18–36 months
  2. Your ageChildren and teenagers typically move teeth faster than adults because their bones are still growing and more responsive to pressure. Adult treatment is absolutely achievable — it simply may take longer and require more patience.
  3. Your complianceThis is the factor most within your control — and the one patients most underestimate.
    • Missing appointments delays treatment. Each missed adjustment means teeth drift back slightly between visits.
    • With clear aligners, not wearing trays 20–22 hours per day is the single biggest cause of extended treatment times. Some patients who were told treatment would take 12 months end up at 20+ months simply because they wore aligners inconsistently.
    • Eating hard or sticky foods with braces can break brackets, which pauses tooth movement until the bracket is repaired.
  4. The type of applianceDifferent orthodontic systems have different biological mechanisms and are suited to different problems. The right appliance for your case — not necessarily the fastest — is always the best choice.
  5. Biology and bone densitySome people’s teeth simply move faster than others. This is partly genetic, partly related to bone density, and partly related to local blood supply. It is not predictable from appearance alone, which is another reason why initial records and monitoring are essential.

  • A removable retainer — worn nightly, indefinitely

Many patients are told they only need to wear retainers for a year or two. In my clinical experience, indefinite night-time wear is the only reliable way to maintain your result long-term. The good news: it becomes completely routine within a few weeks.

Think of retention this way: you spent 18 months investing in your smile. A retainer worn nightly for a few minutes costs nothing and protects that investment permanently. Skipping it risks undoing everything.

How to Keep Your Treatment on Track

The patients who finish on time — or ahead of schedule — consistently do these things:

  • Attend every scheduled appointment without rescheduling
  • Wear aligners for the full 20–22 hours per day if on clear aligner treatment
  • Follow dietary guidelines with braces (no hard, crunchy, or sticky foods)
  • Maintain excellent oral hygiene — inflamed gums slow tooth movement
  • Contact the clinic promptly if a bracket breaks or an aligner is lost or damaged
  • Report any discomfort that persists beyond the first few days after an adjustment

The ones who struggle most are those who were promised an unrealistic timeline by someone who didn’t take proper records — and then felt disappointed when treatment ran longer than expected.
